Where to get it
Free from the government’s immigration forms portal. You can fill it in on screen before printing, which avoids illegible handwriting. Don’t confuse it with EX-15, which is for the NIE number.
Personal details
Name and surnames exactly as on your passport, unabbreviated. NIE, passport, nationality, date and place of birth, parents’ names, marital status and your real address in Spain. A phone and email you actually use.
Card type
Tick first card, renewal, modification, replacement or long-term. It must match the box on fee 790 012; otherwise you have to pay again.
Signing and handing in
By hand, in black or dark blue pen. If you make a mistake, print a new one. For minors, the person with parental authority signs. It is handed in at the appointment, not by post.
